A clinic can exist and a patient can still have no gas money, no ride, and no time off to reach it. The social determinants of health — roads, water, jobs, food, housing, safety — decide outcomes more than buildings do. This Learning Mission teaches the single most useful idea on this site for weighing any health plan: ask what’s around the clinic.

The law behind this guide

CO-50-14 created the Navajo Department of Health and gave it no authority to license physicians and no reach into 638 funding. HIPAA (42 U.S.C. § 1320d-6), the Anti-Kickback Statute (§ 1320a-7b(b)), Stark (§ 1395nn) and the False Claims Act (31 U.S.C. § 3729) are federal. The Nation does not enforce them.

What we found

IHS spends $4,078 per person. Medicare spends $13,185. Federal prisoners: $8,600. (GAO-19-74R.) This is scarcity, not legibility — and it was done to the Nation.

When Congress funds the treaty obligation at the rate it funds prisons.
